Replacement of Lost, Demaged or Stolen Passports

The Consular Section of the Embassy can replace lost or stolen passports with a limited validity passport upon verification of your identity and your citizenship. We will need the following items to process your request for a replacement passport:

Police Report: Issued by local police authorities documenting a loss or theft.

Personal Identification/Proof of Nationality: Personal identity may be established through your driver’s license or other form of photo ID. Proof of nationality may be established through a copy of your lost/stolen passport, birth certificate, or naturalization certificate. If you have no identification, you may bring along an American friend/relative to attest to your U.S. citizenship at the Consulate.

Photographs: Two identical and recent photographs taken full-face measuring 2 inches x 2 inches (5cm x 5cm).

Note: The applicant has to appear in person at the US Consulate and sign the DS-11 form before the Consular Officer.
